Vaccines & Wellness Visits
Q: What is your immunization policy A: You can view our Immunization policy located on our Forms page
Q: When are well visits recommended A: Well visits are recommended more often during the first two years, then once a year starting at age 3 to track growth, development, and overall health.
After-Hours Care
Q: Is there someone I can call after hours A: An on-call nurse is available after 4:30 pm to answer any non-emergency questions. For life-threatening emergencies, you should call 911.
Q: What qualifies as a medical emergency A: A medical emergency is any situation where your child’s life, breathing, or safety is at immediate risk. Examples include severe difficulty breathing, uncontrolled bleeding, seizures, loss of consciousness, or serious injuries. If you believe your child is experiencing a medical emergency, call 911 or go to the nearest emergency room right away.

Q: How do I send in forms or documents A: You can either print the forms out and have them filled out for your appointment, or you can submit forms and documents through our patient portal.
